Fed. Prohibitor - Felony Indictment
18 USC 922 (n) - Persons who are under indictment or information for a crime punishable by imprisonment for a term exceeding one year.
Upon the final court disposition of your charges, you may appeal for relief from the firearm prohibitor. If you were convicted of the charges, you may appeal to the Director of the ISP or to the circuit court, whichever is required by law for your offense. Pursuant to 430 ILCS 65/10(a), the Director of the Illinois State Police (ISP) cannot grant relief for some offenses; however, the FOID Act provides that you may petition, in writing, the circuit court in your county of residence for those offenses. 430 ILCS 65/8 (a) - A person under 21 years of age who has been convicted of a misdemeanor other than a traffic offense or an adjudged delinquent.

430 ILCS 65/8 (b) - A person under 21 years of age who does not have the written consent of his parent or guardian to acquire and possess firearms and firearm ammunition, or whose parent or guardian does not qualify to have a Firearm Owner's Identification Card.

430 ILCS 65/8 (c) - A person convicted of a felony.
Pursuant to 430 ILCS 65/10, the Director of the Illinois State Police (ISP) cannot grant relief for this type of offense, however, the FOID Act does provide that you may petition, in writing, the circuit court in your county of residence when at least 20 years has passed since the end of any term of imprisonment or probation. 430 ILCS 65/8 (e) - A person who has been a patient of a mental health institution within the past 5 years.

430 ILCS 65/8 (f) - A person whose mental condition is of such a nature that it poses a clear and present danger to the applicant, any other person or persons or the community.

430 ILCS 65/8 (g) - A person who is intellectually disabled.

State Prohibitor - Noncitizen Unlawfully in US
430 ILCS 65/8 (i) - A noncitizen who is unlawfully present in the United States.

If you believe your Immigration and Naturalization Service (INS) record is incorrect, contact INS to have your record updated. After your INS is updated you may submit an appeal to the ISP. State Prohibitor - Noncitizen - non-immigrant visa
430 ILCS 65/8 (i-5) - A noncitizen who has been admitted to the United States under a non-immigrant visa.

430 ILCS 65/8 (k) - A person who has been convicted within the past 5 years of battery, assault, violation of an order of protection, or a substantially similar offense in which a firearm was used or possessed.

430 ILCS 65/8 (l) - Persons convicted of domestic battery, aggravated domestic battery, or a substantially similar offense in another jurisdiction.

430 ILCS 65/8 (l) - Persons convicted of domestic battery, aggravated domestic battery, or a substantially similar offense in another jurisdiction.

430 ILCS 65/8 (n) - A person who is prohibited from acquiring or possessing firearms or firearms ammunition by any Illinois State statute or by federal law.

430 ILCS 65/8 (o) - A minor subject to a petition filed under Section 5-520 of the Juvenile Court Act of 1987 alleging that the minor is a delinquent minor for the commission of an offense that if committed by an adult would be a felony.

430 ILCS 65/8 (p) - An adult who had been adjudicated a delinquent minor, under the Juvenile Court Act of 1987 for the commission of an offense that if committed by an adult would be a felony.

430 ILCS 65/8 (t) - A person involuntarily admitted into a mental health facility.

State Prohibitor - Mental greater than 5 yrs, No cert
430 ILCS 65/8 (u) - A person who has had his or her Firearm Owner's Identification Card revoked or denied under subsection ( e ) of 430 ILCS 65/8 or item (iv) of Section 4 of the Firearm Owner's Identification Act because he or she was a patient in a mental health facility, shall not be permitted to obtain a Firearm Owner's Identification Card, after the 5 year period has lapsed, unless he or she has received a mental health evaluation by a physician, clinical psychologist, or qualified examiner and has received a certification that he or she is not a clear and present danger to himself, herself, or others.l
To obtain approval for a FOID card, pursuant to 430 ILCS 65/8(u), submit a Mental Health Certification for Firearm Possession form to the Illinois State Police Firearms Services Bureau. Also, you submit an appeal to challenge the accuracy of the record used to determine your firearm eligibility. State Prohibitor - OP
430 ILCS 65/8.2 - The Department of State Police shall deny an application or revoke and seize a Firearm Owner's Identification Card previously issued if the Department finds that the applicant or person to whom such card was issued is or was at the time of issuance, subject to an existing Order of Protection.
If the order is expired, vacated, or dismissed, call the Customer Service Call Center at 217.782.7980. 430 ILCS 65/4 (2) (i) - The parent or guardian that sponsored your FOID application is ineligible to possess a Firearm Owner's Identification Card.

State Prohibitor - Prohibited from Firearms (OOS IFFS Flag)
430 ILCS 65/8 (n) - A person who is prohibited from acquiring or possessing firearms or firearm ammunition by any Illinois State statute or by federal law.
The Illinois State Police (ISP) cannot take further action on your FOID card until you have obtained relief from the state listed within the denial/revocation letter. 18 USC 922 (g) (9) - Persons convicted in any court of a misdemeanor crime of domestic violence.
The Illinois State Police (ISP) cannot take further action on your FOID card until you have obtained relief from the state listed within the denial/revocation letter. State Prohibitor - Insufficient Funds
430 ILCS 65/5 - The Illinois State Police has determined you are not eligible for a Firearm Owner's Identification Card due to failure to submit the appropriate license fee, pursuant to 430 ILCS 65/5.
To resolve the denial of your FOID card application you must submit a new FOID application. You can submit an application via the ISP website or use the call-in application method by calling the Firearms Services Bureau Customer Service Call Center at 217.782.7980.
State Prohibitor - Mentally Disabled
430 ILCS 65/8 (r) - A person who has been adjudicated as a mentally disabled person.

430 ILCS 65/8 (h) - A person who intentionally makes false statement in the Firearm Owner's Identification Card application.

430 ILCS 65/8 (l) - Persons convicted of domestic battery, aggravated domestic battery, or a substantially similar offense in another jurisdiction.

State Prohibitor - NICS Denied Person (Other agency)
430 ILCS 65/8 (n) - A person who is prohibited from acquiring or possessing firearms or firearms ammunition by any Illinois State statute or by federal law. You have been reported in the Federal Bureau of Investigation, National Instant Criminal History Background Check System (NICS) as a person who is ineligible to possess firearms.
You may challenge the accuracy of the record used in the decision to deny your FOID card by contacting the agency listed above that submitted the disqualifying record to the FBI National Instant Criminal Background Check System (NICS). If that agency concurs with your request and removes the disqualifying records from NICS, you may appeal to the Director of the Illinois State Police. To appeal to the ISP you must submit a FOID Appeal Request, a Request for Investigation and Waiver of Liability and all required documentation as listed in FOID Appeal Requirements. State Prohibitor - NICS Denied Person (NICS)
430 ILCS 65/8 (n) - A person who is prohibited from acquiring or possessing firearms or firearms ammunition by any Illinois State statute or by federal law. You have been reported by the Federal Bureau of Investigation, National Instant Criminal History Background Check System (NICS) as a person who is ineligible to possess firearms.
You may challenge the accuracy of the record used in the decision to deny your FOID card by contacting the FBI National Instant Criminal Background Check System (NICS) that submitted the disqualifying record. If you have additional information (e.g., court documentation, expungement, pardon) to assist NICS in correcting or updating the record, you should include the documentation in your written correspondence to NICS along with your name, date of birth, and contact information. 430 ILCS 65/4 - The Illinois State Police has determined you are not eligible for a Firearm Owner's Identification Card due to failure to adequately complete your FOID card application.
To challenge the accuracy of the information used concerning your denial you may appeal to the ISP. State Prohibitor - Non-resident of IL
430 ILCS 65/8(q) - A person who is not a resident of the State of Illinois, except as provided in subsection (a-10) of Section 4. Please note, if this is the sole basis for the revocation of your FOID card, you may not be prohibited from possessing firearms or firearm ammunition provided that you fall under one of the exceptions listed in 430 ILCS 65/2(b) AND you are not otherwise prohibited from possessing firearms or firearm ammunition under federal law.
A FOID card requires you be a resident of Illinois. After you have obtained a valid Illinois Driver's License or State Identification card submit a new FOID card application. You can submit an application via the ISP website or use the call-in application method by calling the Firearms Services Bureau Customer Service Call Center at 217.782.7980.
State Prohibitor - Firearm Restraining Order
430 ILCS 65/8.2 - The Department of State Police shall deny an application or revoke and seize a Firearm Owner's Identification Card previously issued if the Department finds that the applicant or person to whom such card was issued is or was at the time of issuance, subject to an existing Order of Protection or Firearms Restraining Order.
If the Firearm Restraining Order is expired, vacated, or dismissed, and it has been 1 year or less from the issued date, call the Customer Service Call Center at 217-782-7980 concerning the reissuance of your FOID card.
